Athlete Registration

We are thrilled to announce online registration through our partner, FinalForms. This secure platform offers a simple, user-friendly way to sign up for our programs, enhancing our administrative efficiency and promoting environmental responsibility. By registering through FinalForms., your information is stored in your profile, allowing you to enter your details just once for use across multiple programs, family members, and occasions.

Information To Register

It will be helpful to have the following information handy to allow for accurate completion of your online registration.


  • Doctor’s Name and Number
  • Insurance Carrier Information
  • Emergency Contact Information

All students MUST submit a copy of their most recent physical to the athletics office before the first day of practice. 


Physicals are valid for 13 months from the date of the exam. Students with expired physicals will not be allowed to participate in practices or games until a new physical is provided.

Create Your Account Today

1. Create Parent/ Guardian Account

  • Go to: https://newtown-ct.finalforms.com/
  • Click "New Account": Under the Parent icon and complete the requested information.
  • Click the check box "I'm not a robot"
  • Click "Register"

A confirmation email is sent within 2 minutes. Check your spam if it doesn't arrive. Still missing? Contact support@finalforms.com.

2. Add Your Student(s)

  • Once logged in, click Register Student.
  • Enter Details: You’ll need their legal name, birth date, and—importantly—their school email address.
  • Select Sports: Check the boxes for any sports or activities your child plans to join. This ensures you only see the forms required for those specific activities.

3. Fill & Sign Forms

  • The "Status" Colors: FinalForms uses a simple color-coded system:
  • Red:  Incomplete. Needs your attention.
  • Green: Complete. No action needed.
  • Signatures: At the bottom of each form, type your name exactly as it appears in the signature field to "sign" it.
  • Student Signatures: After you finish the parent portion, FinalForms will automatically email your student at the address you provided so they can sign their own forms.


4. Upload Necessary Documents

  • You upload a photo or PDF of your completed, signed forms into the aproprate sections


Tip: If you have multiple children, once you finish the first one, the system will "pre-fill" about 90% of the info (address, insurance, etc.) for your other kids, saving you a massive amount of typing.
