AVAILABILITY NOTICE

This notice is posted by the Newtown Board of Education in compliance with IRS guidance under sections 6055 and 6056. This notice will remain available on our website through October 15, 2026.

Where to Find Your Form Your 2025 Form 1095-C is now available to view and print for your records via the Employee Access Center.

Why You Aren't Receiving a Copy by Mail Under recent changes to Affordable Care Act (ACA) reporting rules, the Newtown Board of Education is no longer required to automatically mail Form 1095-C to all employees. This form provides details about the health coverage offered to you during the 2025 calendar year.

Note: This form is for your records only. You do not need to file it with your tax return.

How to Request a Paper Copy If you prefer a paper copy, you may request one at any time:

  • Email: hornyakd@newtown.k12.ct.us
  • Subject” 1095-C Paper Copy
  • Details to Include: Please specify how you would like to receive the form (e.g., via mail or interoffice mail or in person pickup).

Once your request is received, your form will be provided within 30 days.
